A very similar accident happened to a former shipmate of mine just a few years ago , only the results were that he had three fingers on his hand crushed, ending his seagoing career. From memory the hatch lids were opened and while the opening wire was being held by the winch he had to climb a short ladder and secure a holding chain to the lid. He secured the chain but the winch walked back while he still had one hand on the hatch coaming track as he was getting back down the ladder.
